Show all options for Single/Multi select in document designer

I have a use case where I have a document that is generated when a form is submitted. There are two fields (one single- and one multi-select) that we must have documented for compliance purposes. We need to see the full list of options and which were selected within the final document. This would be akin to using check boxes in Docusign or Jotform documents.

What specific compliance requirements are you aiming to meet with the documentation of selected options?
Are there any specific formats or templates you prefer for displaying the selected options in the final document?
How frequently do you anticipate needing to update the list of options for these fields?

Jon Darbyshire the requirements are set forth by the state department of education I believe. The courses in question are ones where the course fees are waived for students based on a set of eligibility requirements from the state. So each registered student must select which criteria they meet so that we can receive payment from the state for the course. I've attached a redacted form below with the section marked that is most important. The content comes from a multiselect field. Since the list comes from the state, we don't anticipate it changing that often. On the original paper form, the student had to check next to the option that they fit.
